Activate plugin

Enable "Automatically add UTM tags during creating new form" If need it

Enable "Automatically append UTM parameters to URLs if not exist" If need it

Add default UTM tags for emails

You can also use tokens. Open the list of available tokens

Copy one from tokens list

Insert it

You can use also |transliterate modifier to token to transport it to urlfy format.

You can also add custom tags

You can use tokens too. For example {contactfield=alias}
